If there is a death in the community, the immediate family member should submit this burial form requesting a burial. Scroll down to access the form.
Fill and submit the form to initiate a burial, only one submission is required and you will get a confirmation email after submission. After submission, a NeMC representative will contact you.
Responses to all the questions are required.
